Understanding Out-of-Network Reimbursement

Private-pay clients at Lakeview Counseling & Consulting may request a superbill to submit to their insurance carrier for possible out-of-network reimbursement. This is separate from insurance-based appointments, which use a different scheduling and billing process. For out-of-network reimbursement, clients pay Lakeview directly and then submit documentation to their insurer.

Having out-of-network benefits does not automatically mean that the full session fee — or any specific amount — will be reimbursed.

Reimbursement depends on:

  • Your specific insurance plan
  • Out-of-network eligibility
  • Remaining deductible
  • Your insurer's allowed amount
  • Coinsurance percentage
  • Authorization or referral requirements
  • The type of service provided
  • Your insurer's final claim determination

The out-of-network process:

2Pay for Your Session
3Receive Superbill
4Submit to Insurance

An Estimate Is Not a Guarantee

Any estimate of potential reimbursement is based only on benefit information available at that time. Benefits can change, deductible balances can change, and the information provided by an insurance representative or benefits system may not reflect the insurer's final claim determination.

Your insurance carrier — not Lakeview Counseling and Consulting LLC — makes the final determination regarding coverage, allowed amounts, deductible application, claim approval, and reimbursement.

Key Insurance Terms, Made Simple

Your insurance company can provide the most accurate information about how these terms apply to your specific plan.

Out-of-Network Provider

A provider who does not have a contract with your insurance company. You may still be able to receive services and seek reimbursement, depending on your plan.

Allowed Amount

The amount your insurance company recognizes when processing an eligible service. This amount may be lower than the actual session fee.

Deductible

The amount you may need to pay yourself before your insurance plan begins contributing toward eligible out-of-network services. Some plans have a separate out-of-network deductible.

Coinsurance

After your deductible is met, coinsurance is the percentage of the allowed amount your insurance plan may cover.

Explanation of Benefits (EOB)

A document from your insurance company explaining how a claim was processed. It is not a bill.

Why your reimbursement may differ.

Even with out-of-network benefits, the amount you receive back may be less than expected. Several factors affect the final outcome.

  • Your plan's allowed amount may be lower than the session fee
  • Deductibles reset annually and may not yet be met
  • Coinsurance percentages vary by plan
  • Some plans require prior authorization for mental health services
  • Claim processing timelines and outcomes vary by insurer

A lower-than-expected reimbursement does not necessarily mean something was submitted incorrectly. Insurance plans calculate out-of-network claims according to the terms of the individual policy.

Possible claim outcomes.

Full reimbursement

Your insurer covers the full allowed amount after your deductible is met. Less common, but possible depending on your plan.

Partial reimbursement

Your insurer covers a percentage of the allowed amount. The most common outcome for out-of-network claims.

No reimbursement

Your plan may not include out-of-network mental health benefits, or your deductible may not yet be met.

Superbills for Private-Pay Clients

Private-pay clients at Lakeview Counseling & Consulting may request a superbill to submit to their insurance carrier for possible out-of-network reimbursement. This is separate from insurance-based appointments, which use a different billing process.

Superbill: A superbill is a detailed receipt that includes the information your insurance company may need to process an out-of-network reimbursement claim.

Providing a superbill does not guarantee that your insurance carrier will reimburse any portion of the session fee. Reimbursement depends entirely on your individual plan and the insurer's final claim determination.
